Employee Life Insurance
A base life insurance plan of $25,000 is provided at no cost to employees.
Additional supplemental life insurance of up to 5x your annual salary with a cap of $300,000 is available for purchase. These rates depend on age and are subject to change each year on January 1. Evidence of Insurability may be required.
There is no open enrollment for employee supplemental life insurance. All benefits must be elected within 30 days of your start date.
Spouse Life Insurance
Supplemental life insurance is available for the employee's spouse. Spouses may elect up to 50% of the employee amount elected (and approved) with a maximum of $75,000.
You have 30 days from your hire date to elect this coverage. You have 30 days from the date of marriage to add your spouse to your plan.
Child Life Insurance
You may elect life insurance for dependent children. Cost is a flat rate for all children covered.
$5,000 Coverage - $.75/month for all eligible children.
$10,000 Coverage - $1.50/month for all eligible children.
